directive for the makeup exam

DIRECTIVE ON THE RULES TO BE FOLLOWED BY STUDENTS WHEN RECEIVING AND SUBMITTING REPORTS

1. PURPOSE

This directive has been prepared to determine the rules to be followed by students studying at the Of Faculty of Technology in obtaining and submitting health reports.

 

2. SCOPE

It covers students enrolled in the Of Faculty of Technology at Karadeniz Technical University.

 

3. RESPONSIBLE PARTIES

3.1 Students are responsible for adhering to the principles specified in the directive.

3.2 The Dean is responsible for ensuring the implementation of the directive.

3.3 Department chairs and associate chairs are responsible for implementing the directive.

3.4 Faculty members are responsible for admitting students to makeup exams.

 

4. HEALTH INSTITUTIONS WHERE HEALTH REPORTS CAN BE OBTAINED

4.1 Family physicians

4.2 Health units within institutions

4.3 State hospitals

4.4 Hospitals belonging to the faculty medicine

4.5 Health units affiliated with private institutions

4.6 Private hospitals and other healthcare institutions

 

5. DOCUMENTS TO BE SUBMITTED TO THE DEPARTMENT

5.1 Reports obtained from health institutions specified in the fourth article.

5.2 A form petition prepared for makeup exams and filled out by students.

 

6. TIME FOR THE SUBMISSION OF REPORTS

Students must submit their reports to the Department within FIVE DAYS from the day they were TREATED AS OUTPATIENT or DISCHARGED. Applications made outside of this period will not be processed.

 

7. ACCEPTANCE OF REPORTS, MAKEUP EXAMS, NON-REPORT-BASED EXCUSES

7.1 The decision to hold an excuse exam is made by the relevant department chairmanship. Students who are unable to attend exams due to illness must document their condition with a health report obtained from healthcare institutions. Students whose reports are accepted are granted a makeup exam opportunity only for the missed midterm exams. Even if a student has a report, they are NOT GRANTED A MAKEUP EXAM OPPORTUNITY for final, resit, and graduation exams.

7.2 DAYS ON WHICH REPORTS ARE SUBMITTED ARE NOT COUNTED AS ABSENCES. During the midterm exam period, students with reports are admitted to MAKEUP EXAMS INSTEAD OF MIDTERM EXAMS DURING THE LAST WEEK OF THE SEMESTER if they apply in writing to the Department Chairmanship.

7.3 Students granted a makeup exam opportunity are NOT GRANTED A NEW MAKEUP OPPORTUNITY for any reason.

7.4 If a student's first-degree relatives pass away, face disasters such as floods, earthquakes, fires, enroll late due to additional quotas or horizontal transfers, participate in university sports and cultural activities as officials, or need to take exams such as ÜDS, LES, ÖSS, KPSS, their situation is evaluated by the department chairmanship upon submission of a petition with necessary documents.

7.5 Students who provide false information regarding their report or other excuses, or who attend exams despite having a report, have their exams invalidated, and legal action is initiated against them.

 

8. OTHER PROVISIONS

8.1 Students unable to attend midterm exams due to excuses fill out an "MAKEUP EXAM APPLICATION FORM." The student submits their identity information, the reason for being unable to attend exams, and lists all the courses they couldn't attend exams for on the form along with their report to the department secretariat.

8.2 Whether the report or other excuse is accepted or not is learned from the department secretariat.

8.3 Students whose reports are accepted take the makeup exam on the day, place, and time determined by the Department Chairmanship.
